Course Overview

Chronic diseases represent the main cause of incapacity, mortality and morbidity, in Portugal and worlwide, consuming more than 70% of the health budget. The reality of multimorbidity, especially high in the elderly (60% prevalence in the elderly aged 65 and over) cannot continue to be addressed acording to the traditional, biomedical and disease-centered care without any coordination. This complex reality of health and social care requires a new action based on literacy, self-care, multidisciplinarity and patient-centered care.

Career Prospects
